The Nursing Home Transition & Diversion (NHTD) waiver helps Medicaid-eligible seniors and adults with physical disabilities live in their own home or community setting instead of a nursing facility.
The Traumatic Brain Injury (TBI) waiver provides specialized supports for people with TBI so they can live and recover in their preferred setting with the right care plan.
